The Dementia Care Navigator in Western Sydney, is a service designed to support people from culturally and linguistically diverse backgrounds.
This services provides information on how to connect with Government-funded and other support while seeking a diagnosis and at various stages during the progression of dementia.
Although designed primarily for people who speak a language other than English living with dementia, their family carers and family members, people who are concerned about their memory or worried that someone they know may have dementia can also access support and information.
Benefits include:
- Information sessions to help to navigate Government-funded (My Aged Care, National Disability Insurance Scheme) and other services
- Ongoing one-on-one support in person, via phone and online
- Advice on what services to choose and how to engage with providers
- Access to translated resources and interpretation services
The service also accepts referrals from health, aged care and community professionals. We can support your clients through the complex journeys with My Aged Care, National Disability Insurance Scheme and other service providers. Group sessions for health care and aged care employees will provide information about:

- Dementia Care Navigator service and its benefits
- Service provider referral processes
The Dementia Care Navigator service is free and available to people at all stages of a dementia journey living in the Western Sydney region: Blacktown, Cumberland, Parramatta and The Hills Shire.
