National Younger Onset Dementia Key Worker Program

The National Younger Onset Dementia Keyworker Program:

  • Provides individualised services and support for people living with younger onset dementia, their families and carers.
  • Raises awareness of younger onset dementia through the provision of education to health networks and local community.
  • Builds capacity to meet the needs of younger onset dementia within the disability, aged care, community and residential care sectors through consultation, networking and collaboration with service providers and consumers.

What to expect from the program

Keyworkers act as a primary point of contact for people with younger onset dementia, their carers, families and friends.

They support individuals and families to take an active role in their own health and wellbeing through the provision of information, counselling, advice and advocacy.

Keyworkers also assist individuals to develop goals and action plans, navigate the health system and engage with services appropriate to their own unique needs, i.e. younger-onset specific social clubs, respite, retreats, etc.

To be eligible for Key Worker support, a person must be under the age of 65 when they first make contact with the service or enter the program. This applies to a person who has been diagnosed with younger onset dementia (all forms of), but may also include a person with symptoms of dementia that have not yet been diagnosed depending on an assessment of needs.

Service Providers

Keyworkers are able to assist service providers within the disability, aged care, community and residential care sectors to identify and address gaps relating to the provision of younger onset dementia specific services.

Specialised training and education from Keyworkers enables organisations to modify their services to better meet the needs of people with younger onset dementia while simultaneously fostering collaborative networks within residential and community care sectors.
