The Next Steps: having conversations on life and death. This is a program developed by the former Victorian Quality Council (VQC) to improve clinical communication skills. The Next Steps resources are now overseen by the Victorian Commission for Hospital Improvement.

Advance Care Planning in General Practice in Barwon, Victoria. Barwon Health’s Respecting Patient Choices (RPC) program has collaborated with Barwon Medicare Local and the region’s General Practices to provide advance care planning (ACP) in the clinics. Trained staff from the RPC program facilitate ACP clinics at the practices for patients referred by their GPs. The “ACP Outreach Service” has had a high rate of acceptance, participation, document completion (87%) and satisfaction when ACP is provided through General Practice.
