Free online Advance Care Planning Toolkit
A free DIY Toolkit created by Peninsula Health is available online. The toolkit enables you to follow the prompts to fill out a Medical Power of Attorney form and to create your personalized Advance Care Directive. Your forms are then saved to your computer and printed for signing. These online tools are available to everyone, you don’t have to live in the Peninsula Health area to use them.
