We are looking for people to complete a 15-20 minute survey that asks for your opinions about your risk of developing diseases such as dementia, heart attack, stroke and diabetes. It will also explore whether you would want to know about your risk and how you would prefer to learn about it.

To participate in this study, you must live in Australia and be aged between 40 and 79 years. If you complete the survey, you may choose to enter a draw to win one of ten $20 gift vouchers.

This study will provide important information for further work that the research team is doing to develop questionnaires that can assess your risk of developing dementia, myocardial infarction (heart attack), stroke and diabetes. This will help to ensure that the questionnaires are more user-friendly and acceptable to people aged 40-79.
