ENgaginG older people Across transitions of care to improve medication manaGEment (The ENGAGE Project) – Developing and Testing Co-designed Strategies

This project aims to enhance engagement in medication management during transitions of care. Through facilitator-led workshops, participants will explore and reflect on various activities related to medication use and safety. These workshops will provide valuable insights for researchers to design effective interventions that improve patient collaboration, enhance communication, and ultimately increase medication safety during care transitions.

This study has been approved by the Monash University Human Research Ethics Committee (approval no. 46801)

Participation

People who meet the following criteria may be eligible to participate: 

  • older adults (aged >65 years) who have previously been admitted to hospital
  • family member of an older adult and assist with their medicines 
  • access to a computer or phone that allows Zoom access 
  • able to provide informed consent 

Participants will be reimbursed with a $50 Coles/Myer voucher as a thank you for their time.
