This project will use a multidisciplinary approach, involving exercise, nutritional health, social health, and medication optimisation, to determine if these factors can slow the rate at which older adults transition from a pre-frail to a frail state. In essence, the program is aimed at increasing the quality of life and functional capacity of older adults, with the aim to allow individuals to maintain their functional independence. 

This study has been reviewed and approved by the Metro North Health Human Research Ethics Committee (EC00172) approval HREC/2023/MNHA/102635

Participation

People who meet the following criteria, may be eligible to participate; 

Inclusion criteria includes:

  • Visited a geriatrician or GP within the past 12 months;
  • Aged ≥65 years, or ≥55 years if Aboriginal or Torres Strait Islander;
  • Living in the community (i.e., not in aged care or hospitalised);
  • Have access and ability to use a computer or other electronic device with internet service; and
  • Be able to travel to the exercise professional training sessions of their state (at Pymble, NSW).

If the above criteria are met, a screening call will be made to determine eligibility. 

Participation details:

Individuals who participate will be randomly allocated to either a supervised or self-directed group, which will address the four multidisciplinary pillar over a trial period of 6 months. 

The supervised group will be expected to undertake:

  • 2 exercise sessions/week (supervised by Accredited Exercise Physiologists), 
  • 1 consultation with and a customised program delivered by an Accredited Practicing Dietitian, 
  • A customised health behaviour program with regular contact from a health coach,
  • An opportunity to optimise the individual's current medication regime
  • Optional group sessions to promote social support

The self-directed group will have access to online frailty resources.
